Connections by Maryvale provides coordinated behavioral health services that support youth and families throughout Los Angeles County. Through prevention, early intervention, and Enhanced Care Management, we help young people build resilience, improve well-being, and connect to the services they need to thrive.
Program Overview
Connections by Maryvale supports youth and families across Los Angeles County through coordinated care, prevention, and wellness services. Through its two programs—CARE CONNECT and YOUTH CONNECT—Connections helps participants navigate healthcare and community resources while building the skills, resilience, and support systems needed to thrive. Together, these programs provide a continuum of care that promotes lasting wellness and positive outcomes for children, youth, and families.
CARE CONNECT is Maryvale’s Enhanced Care Management (ECM) program, providing comprehensive, whole-person, community-based care coordination for eligible youth and their families. Our ECM Lead Care Managers partner with families to identify their strengths and needs, develop individualized care plans, coordinate services across healthcare providers, schools, behavioral health, and community organizations, and help ensure families can successfully access the support they need.
Services are delivered wherever they are most effective—including in the home, at school, in the community, at a clinic, or through telehealth when appropriate and requested by the family. CARE CONNECT is designed to reduce barriers to care, improve communication among providers, and support long-term health, behavioral health, and overall well-being. By building trusted relationships with youth and caregivers, our team helps families navigate complex systems, connect to essential resources, and achieve their personal goals.
YOUTH CONNECT is Maryvale’s prevention and early intervention program, empowering youth and young adults ages 13–24 to build resilience, strengthen mental wellness, and make healthy life choices. Funded through the Project Youth Wellness (PYW) initiative in partnership with Azusa Pacific University, the program provides evidence-informed prevention education, early intervention, and connections to community resources. We partner with local school districts, community wellness centers, transitional age youth (TAY) programs, teen centers, and other community organizations to deliver services in person and virtually, increasing access to care where youth live, learn, and connect. Services include:
- Mental health and emotional well-being
- Substance use prevention, education, and early intervention
- Life skills and resilience building
- Care coordination, referrals, and resource navigation
- Virtual Prevention and Early Intervention Groups
As part of the program, Maryvale offers a free 8-week online Prevention & Education Group for teens ages 12–17. Held weekly, the group gives teens a supportive, judgment-free space to build coping skills, connect with peers, and get rewarded for participation, one part of the broader YOUTH CONNECT continuum of care.

- Youth and families who qualify for Enhanced Care Management under Medi-Cal/CalAIM
- Families seeking coordinated support to navigate care, resources, and services
- Youth and caregivers who benefit from consistent, hands-on care coordination
- Youth ages 13–24 served through Project Youth Wellness
- Teens ages 12–17 referred to the Prevention & Education Group
