JOB SUMMARY::
Reporting to the Director of Connections by Maryvale, the Intake/Outreach Coordinator plays a vital role in managing the referral, eligibility, and intake process for Connections by Maryvale. This position involves conducting interviews, enrollment, tracking and monitoring referrals and leads, eligibility, coordinating with various stakeholders, assigning cases, utilization of an electric health records system and case relations manager system, and ensuring accurate documentation. The Intake/Outreach Coordinator also collaborates with community partners, performs outreach activities, represents Connections in community events, and assists in data compilation for program reports.

- Utilize Trauma Informed Care approach in recognizing the presence of trauma symptoms and acknowledge the role that trauma has played in the lives of the clients/residents.
- Assist in the healing process by developing appropriate RICH® (Respect, Information, Connection and Hope) relationships with those served.
- Follow Universal Precautions when exposed to blood and body fluids.
- Follow Occupational Safety Health Administration (OSHA) regulations pertaining to safety and the use of equipment/machinery.
- Support Maryvale’s Performance and Quality Improvement Plan and participate in discussion regarding department or program goals, data collection and/or analysis.
- Review and promptly respond to all intake inquiries, including assessing basic eligibility for services.
- Conduct interviews, either in person or over the phone, to determine suitability for services and appropriate level of care.
- Collect additional information and documentation from parents/guardians, commercial and/or Medi-Cal insurance, Department of Probation, schools, or other sources as needed to facilitate the intake process.
- Coordinate with the billing department to address eligibility and related matters.
- Collaborate with the Director of Connections by Maryvale to assign cases and identify suitable funding/program types.
- Participate in weekly disposition meetings with the Director of Connections and the Connections team to evaluate referrals and discharges.
- Assist in completing and organizing intake/enrollment documentation for clinicians, whether in the office or field setting.
- Follow up on non-responsive referrals and coordinate with the referring parties accordingly.
- Manage vacancies and client rosters across various programs.
- Maintain collaborative relationships with referring parties, including schools, health plans, contracts, providers, care managers, and community partners, to ensure a consistent flow of referrals.
- Will be knowledgeable about all Maryvale Programs and collaborate with all Programs to ensure that outreach and referrals are being screened for all eligible programs at Maryvale.
- Engage in community outreach efforts to promote services and establish connections, such as participating in community events and collaborative meetings.
- Work with the Director to manage requests, establish relationships, and provide requested information to Commercially insured Health Plan partners.
- Compile data for monthly reports on referrals, intakes, and discharges for all Connections by Maryvale pillars of services.
- Collaborate with the Director of Connections to review and monitor electronic client charts, updating the Client Caseload sheet with current documents for all cases.
- Respond to general phone/website inquiries regarding services, resources, and other relevant information.
- Complete initial assessments and/or maintain a small clinical caseload as needed and based on staff eligibility (registered with the BBS).
- Conduct initial screenings and assessments for potential program participants.
- Connect individuals seeking treatment with appropriate resources and support services.
- Conduct outreach efforts to promote the program's services and increase community awareness.
- Maintain accurate and up-to-date records of all interactions and services provided.
- Perform other duties as assigned/requested.
QUALIFICATIONS:
To be successful in the role of Intake/Outreach Coordinator for Connections by Maryvale Program, you should possess the following qualifications:
- Bachelor's degree in a relevant field such as Social Work, Psychology, Counseling, or a related discipline.
- Excellent verbal and written communication skills to effectively interact with individuals seeking services, referring parties, and internal staff.
- 1-2 years of experience in a similar role within a Co-Occurring Disorder Program or a related mental health setting.
- Familiarity with the referral and intake process is highly desirable.
- Experience with Enhance Care Management Services is highly desirable.
- Proficiency in Microsoft Office applications, including Word, Excel, and Outlook.
- Typing speed of at least 45 words per minute with accuracy.
- Bilingual fluency in English and Spanish is required due to the diverse population served.
- Possession of a valid California Driver's License.
- Strong organizational skills with the ability to manage multiple tasks, prioritize effectively, and meet deadlines.
- Ability to build and maintain collaborative relationships with various stakeholders, including parents/guardians, referral sources, community partners, and internal staff.
- Experience in compiling and managing data for reports and maintaining accurate records.

JOB SUMMARY:
Under the direct supervision of the ECE Administrator, Associate Teachers (AT) are responsible for providing a warm, caring, calm, orderly, safe and healthy early learning environment in accordance with the policies and procedures of the Early Education program. ATs may be assigned to work in a classroom with children birth through 12 years or as a support teacher across all age groups. ATs are required to provide high quality care, supervision and educational experiences to all children. ATs are responsible for the day-to-day supervision of Teacher’s Aides, as assigned to their classroom.

ESSENTIAL JOB DUTIES:
- Support Maryvale’s Performance and Quality Improvement Plan and participate in discussion regarding department or program goals, data collection and/or analysis.
- Plan and organize engaging activities to ensure a safe, engaging, and positive early learning environment to meet the emotional, social, physical and cognitive needs of each child.
- Arrange and create a developmentally appropriate learning environment in accordance with program goals and Maryvale’s philosophy that is compliant with ERS at a level 5 rating or higher.
- Provide engaging, developmentally appropriate and culturally diverse daily activities for the children in accordance with Creative Curriculum. Where applicable, planning and implementation of activities is to be a collaborative effort amongst the teaching team.
- Provide virtual learning opportunities daily through various methods to maintain the developmental progress of currently enrolled children. These methods should include but are not limited to: email, online learning platforms, and video. Maryvale will post videos online via social media platforms managed by the agency and via the agency website. Maryvale owns and maintains all rights for educational content created for the purposes of educating children and families or advertising our Early Education Programs. Content may continue to be posted after an Associate ends their employment with Maryvale.
- Ensure that children assigned to his/her care are visually supervised at all times, in accordance with Maryvale policy and procedure and Title 22 regulations.
- Ensure that health and safety standards and procedures are effectively executed including but not limited to: daily sanitation, diaper changing, record keeping/daily reports, safe sleep, safety checklists, adherence to allergy lists and food program documentation at point of service.
- Meet with parents for conferences at 60 days, and every 6 months thereafter for review of DRDP and periodically as needed. Ensure that family inquiries are responded to professionally and in a timely manner.
- Communicate with families in accordance with the Maryvale communication policy including but not limited to face to face, via email, video conference and telephone to monitor, inform and support families.
- Complete 21 hours of professional development annually. Upload said professional development into the Workforce Registry as trainings/coursework are completed.
- Attend and actively contribute to: in-services training, staff meetings, discussion groups.
- Maintain a current Child Development Permit at the Associate Teacher level or higher. ATs must hold a current permit to be eligible for their role, and will be demoted, suspended or dismissed if their permit is not maintained. Associate Teacher permits are only renewable one time, and require an additional 15 units of coursework towards a Teacher permit.
- Maintain regular records on children e.g. daily reports home, anecdotals, behavior logs, DRDPs.
- Communicate immediately with supervisor and/or Director regarding concerns or questions.
- Communicate with the appropriate EEC Administrator and/or VP of ECE immediately regarding any issues regarding program or child safety – Maryvale teachers are mandated reporters.
- Provide day-to-day supervision, mentorship and training of Teacher’s Aides as assigned to the classroom, and provide regular feedback to the ECE administrative team regarding the job performance of said Teacher’s Aides.
- Work in cooperation with fellow Associates to strive for team cohesiveness.
- Exhibit compassionate service, utilizing patience and empathy.
- Maintain confidentiality including but not limited to sharing with parents/guardians any personal information regarding children other than their own.
- Use common sense and good judgement to act in accordance with Maryvale policy and procedure. Problem solve and make sound judgments in all areas affecting the care and supervision of the children entrusted to the care of the program.
- When necessary, call for assistance and respond to emergency situations accordingly. In an emergency staff are to remain on campus until all children have left the premises OR they have been specifically authorized by the Center Director or VP of ECE to leave the premises.

QUALIFICATIONS:
- Current Immunizations.
- Active Fingerprint Clearance.
- Child Development permit issued by the California Commission on Teacher Credentialing (CTC) at the Associate teacher or higher level. Permits must be kept current, and cannot be allowed to lapse or expire.
- Associate Teachers are responsible for maintaining a professional development plan, which includes setting professional growth goals, maintaining 21 hours of professional development annually, and renewing/upgrading their permit with the CTC 3-6 months prior to expiration.

Medical, Dental & Vision Insurance - Premiums for Associates are covered 100% by the agency. Maryvale offers HMO medical, DHMO dental and a vision plan for full-time Associates. Holiday Pay - Maryvale recognizes 10 full-day paid holidays, and 1 Holiday of Choice each calendar year (Regular part-time employees receive pro-rated time).
Vacation - Maryvale recognizes the value of rest and relaxation and encourages Associates to take time off with their families. Regular full-time Associates accrue paid vacation leave according to the following schedule:
- 1 to 3 years of service - 10 working days per year
- 4 to 7 years of service - 15 working days per year
- 8 years or more of service - 20 working days per year
Sick Time - Associates accrue sick time at 1 hour for each 28.9 hours of work.
Life Insurance - Maryvale pays for a basic Life & Accidental Death & Dismemberment (AD&D) Insurance Policy that pays beneficiaries a flat $25,000 death benefit.
401(k) Pension Plan - Associates are eligible to enroll in the 401(k) Retirement Plan on the first day of the month after their hire date.
- Match of Employee Contributions: After completing one year of service, Maryvale will match 100% of an Associate’s salary deferral up to a maximum of the first 3% (1%-3%). In addition, Maryvale will match 66% of an Associate’s salary deferral up to a maximum of the next 3% (3%-6%).
- Non-Elective Contribution: After an Associate completes one full year of service, Maryvale on the Associate’s behalf, will begin making a fixed non-elective contribution equal to 3% of the Associate’s income. This contribution is separate from the matching contribution and vests over a 5 year period.
Long Term Disability Insurance - Maryvale offers Long Term Disability Insurance to all benefits-eligible Associates at no cost to the Associate. Maryvale pays the full premium for these benefits which provide salary protection in the event an Associate becomes disabled for an extended period of time.
