EDUCATION CASE MANAGER

Under the general supervision of the Social Work Coordinator, the Education Case Manager is responsible for collaborating with the “core treatment team.” Additionally, the Education Case Manager is responsible for overseeing and facilitating the school enrollment and registration of all Residential clients and serve as a liaison with the school districts.

 

ESSENTIAL JOB DUTIES:

  • Create an atmosphere of enthusiasm and energetic commitment and serve as a role model in carrying out the Mission.
  • Promote and support the attitudes, behaviors, knowledge and skills necessary to work respectfully and effectively with each other as well as demonstrate the ability to address the needs of the children and families Maryvale serves.
  • Follow the Trauma Informed Care philosophy when supporting and addressing the needs of the children and families Maryvale serves.
  • Utilize Trauma Informed Care approach in recognizing the presence of trauma symptoms and acknowledge the role that trauma has played in the lives of the clients/residents.
  • Assist in the healing process by developing appropriate RICH® (Respect, Information, Connection and Hope) relationships with those served.
  • Follow Universal Precautions when exposed to blood and body fluids.
  • Follow Occupational Safety Health Administration (OSHA) regulations pertaining to safety and the use of equipment/machinery.
  • Support Maryvale’s Performance and Quality Improvement Plan and participate in discussion regarding department or program goals, data collection and/or analysis.
  • Attends case conferences, discharge planning meetings, and other meetings as assigned to determine the case management needs of clients and their families.
  • Responsible for assisting in the development of comprehensive treatment plans.
  • Responsible for documenting progress through completion of Department of Mental Health documentation and providing feedback to treatment team members for inclusion in group home contract compliance documentation.
  • Adhere to documentation standards set for the by the Department of Mental Health (DMH) as it relates to providing Mental Health Rehabilitation Services.
  • Expected to provide Individual Rehabilitation/ Life Skills Services/ workshops as indicated to an assigned caseload.
  • Adhere to the 24-hour turnaround requirement for client progress notes.
  • Responsible for demonstrating cultural sensitivity when interfacing with clients and families.
  • Attends all Residential Department meetings and in-service trainings.
  • Serves as a liaison for Maryvale’s Residential and the public schools and school districts.
  • Ensures the timely school enrollment of all Maryvale Residential clients within 3 school days as required by the DCFS Group Home Agreement.
  • Assist with notifying if resident is not in proper school attire.
  • Ensures the timely school registration of all Maryvale Residential clients.
  • Understands AB490 and can advocate for the educational rights of Maryvale students.
  • Attends and is an active participant in IEP’s for Maryvale residents.
  • Attend all school related meetings, including behavioral issues, SST, SARTs, AB216 and IEP’s.
  • Monitors school attendance and coordinates with the Group Supervisor and Therapist to resolve attendance problems and behavior problems.
  • Works collaboratively with the public schools and Non-Public Schools and the Maryvale treatment team to address academic needs.
  • Work with HSO and school nurse regarding medication issues.
  • Notify school of any allergies and special diets.
  • Assist with specialized programs activities.
  • Provide supervision, protection and care of the children individually and in groups at all times.
  • Provide assistance to each child in working with a group and in handling individual problems.
  • Administration of discipline and setting of limits for behavior.
  • Notation of the child’s progress; identification of the possible need for professional services; and communication of such findings to professional staff.
  • Provide assistance in dressing, grooming, bathing and other personal hygiene.
  • Assistance with taking medication, as necessary.
  • Ensure that medication shall be kept in a safe and locked place that is not accessible to persons other than associates responsible for the supervision of the centrally stored medication.
  • Ensure assistance is provided to the children in meeting their medical and dental needs.
  • Maintenance of house rules for the protection of clients.
  • Supervision of client schedules and activities.
  • Collaborating with outside resources including Title 1 for tutoring services.
  • Performs other duties as assigned.

QUALIFICATIONS:

  • Bachelor’s Degree in Social Work, Psychology, Counseling, or a related field.
  • Four years’ experience in a mental health setting.
  • Must be proficient in Microsoft Office products, including Word, Excel, and Outlook and 45wpm accuracy.
  • CPR and First Aid trained; Pro-Act trained.
  • Possess the ability to communicate effectively through oral and written skills.
  • Possess case management experience working with SED children.
  • Bilingual (English/Spanish) preferred.
  • Valid California Driver’s License.

Medical, Dental & Vision Insurance - Maryvale offers a choice of two medical plans and two dental plans for full-time Associates. In addition, we offer vision insurance through VSP. Holiday Pay - Maryvale recognizes 10 full-day paid holidays,  and 1 Holiday of Choice each calendar year (Regular part-time employees receive pro-rated time).

Vacation - Maryvale recognizes the value of rest and relaxation and encourages Associates to take time off with their families. Regular full-time Associates accrue paid vacation leave according to the following schedule:

  • 1 to 3 years of service - 10 working days per year
  • 4 to 7 years of service - 15 working days per year
  • 8 years or more of service - 20 working days per year

Sick Time - Associates accrue sick time at 1 hour for each 28.9 hours of work.

Life Insurance - Maryvale pays for a basic Life & Accidental Death & Dismemberment (AD&D) Insurance Policy that pays beneficiaries a flat $15,000 death benefit.

401(k) Pension Plan - Associates are eligible to enroll in the 401(k) Retirement Plan on the first day of the month after their hire date.

  • Match of Employee Contributions: After completing one year of service, Maryvale will match 100% of an Associate’s salary deferral up to a maximum of the first 3% (1%-3%). In addition, Maryvale will match 66% of an Associate’s salary deferral up to a maximum of the next 3% (3%-6%).
  • Non-Elective Contribution: After an Associate completes one full year of service, Maryvale on the Associate’s behalf, will begin making a fixed non-elective contribution equal to 3% of the Associate’s income. This contribution is separate from the matching contribution and vests over a 5 year period.

Long Term Disability Insurance - Maryvale offers Long Term Disability Insurance to all benefits-eligible Associates at no cost to the Associate. Maryvale pays the full premium for these benefits which provide salary protection in the event an Associate becomes disabled for an extended period of time.
